The Heavy's Sandvich was first seen in Meet the Scout and has since been given a movie of its very own. It consists of two triangular slices of bread with lettuce, tomatoes, cheese, ham, and bologna in between. The Sandvich is an unlockable replacement for the Heavy's shotgun once 10 Heavy achievements are earned. When eaten, it replenishes 300 HP over four seconds. The decision to eat a Sandvich must not be taken lightly; the Heavy is completely vulnerable and very loud during the eating process ("OM NOM NOM OM NOM"). The Heavy doesn't use up his Sandvich when he eats it, so his only real concern is being caught short while enjoying the delicious edible device.
In terms of weapon choice, the Sandvich is a great tool for area defense, as it gives the Heavy the ability to replenish health between enemy waves without having to abandon his post. Similarly, an offensive Heavy without a Medic can step back from the front line and grab a quick snack before resuming his rampage. However, enemies at range are far more dangerous, especially Snipers, and if the Heavy runs out of Minigun ammo with the Sandvich equipped, he is left with only melee for offense. Note that you cannot use the Sandvich while underwater, as this will cause the Sandvich to become wet and unsavory. The healing given by a Sandvich, not surprisingly, does not extinguish fire, though it can save a Heavy with critically low HP as it heals faster than the fire can burn him.
Heavies who are feeling generous can hit the Alternate Fire Button (Mouse2 by default) to drop the Sandvich at his feet for others to pick up. The dropped Sandvich heals 50% of the collector's HP. Since the sandvich acts exactly like a medium medkit, The Heavy who dropped the Sandvich cannot use it again until he picks up a Medkit at Full Health, or returns to the Resupply locker. If the Heavy switches classes, if he uses the Sandvich's alternate fire again, or if he just waits 30 seconds, the dropped Sandvich will disappear.

Trivia
- The Sandvich is listed as a Level 1 Lunch Box in the loadout menu.
- A Heavy holding the Sandvich drops no metal when killed. Sandviches dropped in this way can be collected by a Scout to gain 75 HP and earn the "Dodgers 1, Giants 0" Achievement. Any other class that collects a Sandvich this way gains only 50 HP.
- The Sandvich was first portrayed on the "Meet the Scout" video, before it was even released.
- The Sandvich has no unique taunt. Taunting while the Sandvich is held is the same as pressing the primary attack.
- Interestingly, there are 17 unique sounds for a possibly scrapped Sandvich taunt in the TF2 GCF.
- As with all weapons, there is a slight delay between switching to the Sandvich and left-clicking (or whatever your 'shoot' button's attached to) to perform a primary attack. However, the taunt key (G is the default key) has no weapon switching delay. Since both left-clicking and taunting achieve the same goal for the Sandvich, a heavy should use the taunt key (G) rather than the primary attack key (left-click) as this allows the heavy to immediately start the eating taunt with the Sandvich after switching to it, bypassing the delay and minimizing the time you are vulnerable.

- If a Heavy eats the Sandvich constantly, it is possible to receive a headshot several times without dying.
- Despite the Sandvich being made of bread, meat, cheese, and vegetables, it can still be crafted into scrap metal.
- Whilst eating the Sandvich the player in question can stop the chewing sounds the Heavy makes and gives his vulnerability away, by using one of the voice commands from the voice menu. For example: "Go. Go. Go." or "Jeers".
- Despite it's popularity, critical Sandviches do not receive any extra bonuses or boosts apart from the aesthetic critical effects.
